Aware Recovery Care Georgia implements an innovative "Recover Where You Live" model for addiction treatment aimed at adults aged 18 and older. Their in-home addiction treatment (IHAT) program blends home-based and virtual support, along with concierge services tailored to fit individual requirements. The center employs a biopsychosocial framework that incorporates evidence-based treatments, medication-assisted therapy (MAT), personal and family therapy, as well as collaboration with outside healthcare providers. The guiding principles of Aware Recovery Care revolve around empathy, comprehension, and the conviction that every individual possesses the right to attain sustainable recovery. By catering to the unique circumstances of each client and offering consistent support in their living environment, they aim to lead individuals toward a healthier, more satisfying, and addiction-free future. Each client is assigned a care coordinator (either a nurse or social worker) along with two client recovery advisors (CRAs) who provide peer support and serve as facilitators throughout the recovery process. They help identify necessary external care and manage appointments with primary care practitioners, therapists, medical experts, and family education specialists. If clients are currently working with these professionals, Aware Recovery Care’s medical director ensures seamless communication and collaboration. The family education facilitator assists family members by providing knowledge on addiction, recovery strategies, and collective healing. The program at Aware Recovery Care is adaptable, extending monthly, with treatment durations varying from one month to a year. It includes up to 20 virtual one-on-one therapy sessions with an Internal Psychotherapist, during which clients learn techniques for stress management and enhancing self-esteem, self-compassion, and communication abilities. Moreover, negative thought patterns are addressed and transformed. A regular day at Aware Recovery Care features an hour-long interaction with a member of the care team, complemented by either an in-person meeting with one of the two CRAs or a virtual consultation with their one-on-one therapist. Clients also have monthly check-ins with their Care Coordinator. The care team consistently collaborates to ensure that treatment remains aligned with the clients’ evolving needs and objectives. Upon completion of the program, clients at Aware Recovery Care participate in a personalized discharge strategy designed specifically for their individual treatment goals and home situation. This process involves a discussion with their clinical coordinator about continued engagement with social support networks, setting future goals, and scheduling follow-up appointments with licensed practitioners who supported them during their recovery journey.
