Aware Recovery Care Indiana implements an innovative “Recover Where You Live” model for addiction treatment aimed at adults aged 18 and older. Their in-home addiction treatment (IHAT) program merges personalized at-home services with virtual care and concierge support to address specific needs. This comprehensive approach to addiction includes a biopsychosocial framework featuring evidence-based therapies, medication-assisted treatment (MAT), individual and family therapy, as well as coordinated support from external providers. For those requiring detoxification, medically supervised detox is facilitated via virtual nursing care and MAT. The core philosophy at Aware Recovery Care is rooted in compassion and the understanding that every person is entitled to the chance for lasting recovery. By catering to the distinct needs of each client and offering constant support within their living environment, they aim to assist individuals in transitioning to healthier, more fulfilling lives free from addiction. Each participant at Aware Recovery Care is assigned a care coordinator, either a nurse or social worker, along with two client recovery advisors (CRAs) who provide peer support and coordination throughout the treatment process. They help identify any external care needed and set up appointments with primary care providers, individual therapists, and various medical specialists. If clients have existing healthcare professionals, Aware Recovery Care’s medical director collaborates with them to ensure cohesive care. Additionally, a family education facilitator is available to support family members, providing them with insights on addiction and recovery while guiding them in healing together. The program adjusts monthly, with treatment durations that can range from one month to a full year. Clients can access up to 20 individual virtual therapy sessions with an Internal Psychotherapist, during which they enhance their skills in stress management, self-esteem, self-compassion, and communication, while also working to alter negative thinking patterns. On a typical day at Aware Recovery Care, clients enjoy an hour-long visit from a member of the care team, along with either an in-person meeting with one of the two CRAs or a virtual session with their assigned therapist. Monthly check-ins with the Care Coordinator are also part of the routine. The care team frequently collaborates to ensure that treatment remains aligned with the clients' evolving needs and objectives. When it comes to discharge, clients at Aware Recovery Care experience a customized process that reflects their specific treatment plan and home context. The clinical coordinator engages them in discussions about their discharge plan, emphasizing continued connection with social supports, defined goals, and scheduling further appointments for aftercare with licensed providers who were part of their recovery journey.
