By focusing on the specific needs of each client and offering ongoing assistance in the comfort of their home, the center aims to lead individuals toward a healthier, more fulfilling, and addiction-free lifestyle.
Each participant is assigned a care coordinator—either a nurse or social worker—alongside two client recovery advisors (CRAs) who serve as peer support and guides throughout the treatment process. These advisors assess external care requirements and facilitate appointments with primary care providers (PCPs), individual therapists, medical experts, and family education specialists. When clients are already working with these professionals, the medical director at Aware Recovery Care ensures seamless coordination of care. A family education facilitator is also available to support loved ones, providing essential information about addiction and recovery to promote collective healing.
The program at Aware Recovery Care is structured on a monthly basis, offering treatment durations that span from one month up to a year. Their combined in-home and virtual services include up to 20 individual therapy sessions with an Internal Psychotherapist. During these interactions, clients are equipped with tools to manage stress, enhance self-esteem and self-compassion, and refine their communication abilities. Additionally, clients confront and work to amend detrimental thought patterns.
In a typical day at Aware Recovery Care, clients can expect an hour-long meeting with a care team member, complemented by either an in-person visit from one of the two CRAs or a virtual session with their individual therapist. Monthly check-ins with the Care Coordinator are standard, and the treatment team consistently collaborates to ensure that support remains aligned with each client’s needs and objectives.
As clients approach the end of their treatment, Aware Recovery Care implements a customized discharge process tailored to their specific plan and living situation. They review a detailed discharge strategy with their clinical coordinator, which includes maintaining connections with social support networks, setting ongoing goals, and scheduling aftercare appointments with licensed providers involved in their treatment journey.
