About
Aware Recovery Care Connecticut utilizes a distinctive “Recover Where You Live” model for treating addiction in adults aged 18 and older. Their in-home addiction treatment (IHAT) program offers a blend of at-home and virtual care, coupled with personalized concierge services to cater to individual requirements. Addressing addiction through a biopsychosocial framework, Aware Recovery Care incorporates evidence-based therapies, medication-assisted treatment (MAT), personal therapy sessions, family counseling, and coordination with external healthcare providers. For those in need, clients can undergo a medically supervised detox via virtual nursing assistance and MAT.
The treatment philosophy of Aware Recovery Care is rooted in empathy, awareness, and the conviction that everyone deserves a chance to achieve enduring recovery. By focusing on the distinct needs of each individual and providing continuous support within their home environment, they aim to lead clients toward a healthier, more fulfilling life free from addiction.
Each client at Aware Recovery Care is assigned a care coordinator—either a nurse or social worker—and two client recovery advisors (CRAs) who serve as peer supports throughout the treatment journey. They help identify necessary outside care and organize appointments with a primary care physician (PCP), personal therapist, medical specialists, and a family education facilitator. If clients have existing healthcare professionals, Aware Recovery Care’s medical director works to coordinate care with them. The family education facilitator offers support to families, helping them understand addiction, recovery, and ways to heal together.
The program at Aware Recovery Care is available on a monthly basis, with treatment durations varying from one month to a year. The in-home and virtual offerings include as many as 20 one-on-one therapy sessions with an internal psychotherapist. During these sessions, clients work on managing stress, enhancing self-esteem and self-compassion, and improving communication skills while addressing and transforming negative thought patterns.
A typical day in the Aware Recovery Care program features a one-hour visit from a team member, complemented by either an in-person meeting with a CRA or a video consultation with their personal therapist. Clients also have monthly check-ins with their care coordinator, ensuring that the care team remains aligned with their needs and treatment objectives.
The discharge process at Aware Recovery Care is individualized, designed specifically to fit each client's treatment plan and home situation. Clients engage in discussions about their comprehensive discharge strategy with their clinical coordinator, focusing on maintaining social connections, achieving set goals, and scheduling aftercare appointments with licensed providers from their treatment experience.
