Aware Recovery Care Rhode Island utilizes an innovative “Recover Where You Live” methodology for addressing addiction challenges. Their in-home addiction treatment (IHAT) program merges at-home support with virtual assistance from a dedicated treatment team tailored to meet the specific needs of each individual. Aware Recovery Care tackles addiction using a biopsychosocial framework that incorporates evidence-based therapies, medication-assisted treatment (MAT), psychiatric assessments, individual and family therapy, as well as coordinated support from external healthcare providers. Each client at Aware Recovery Care is assigned a care coordinator, such as a nurse or social worker, along with two client recovery advisors (CRAs) who serve as peer support and guides throughout the treatment journey. They assess the need for external services and help schedule appointments with primary care physicians, individual therapists, medical experts, and facilitators for family education. If clients are connected with these professionals already, Aware Recovery Care’s medical director ensures coordinated care. The family education facilitator plays a crucial role in supporting and educating family members about addiction and the recovery process. The comprehensive 52-week (or shorter) program from Aware Recovery Care encompasses up to 20 virtual one-on-one therapy sessions with a qualified Internal Psychotherapist. These sessions focus on stress management and the enhancement of self-esteem, self-compassion, and communication abilities, while also promoting the identification and alteration of negative thinking patterns. A psychiatric evaluation performed by an external provider assesses clients’ mental health needs and any beneficial medications. On a typical day at Aware Recovery Care, clients engage in a one-hour session with a member of their care team, supplemented by either an in-person visit from one of their two CRAs or a virtual meeting with their one-on-one therapist. Clients also have monthly meetings with their Care Coordinator. The treatment team collaborates regularly to ensure that care remains aligned with the clients’ evolving needs and objectives. As clients approach the end of their program at Aware Recovery Care, they participate in a customized discharge process that reflects their individual treatment plan and home situation. This process includes a discussion with their clinical coordinator regarding their detailed discharge plan, which considers ongoing involvement within their social circles, goals established during treatment, and aftercare appointments set with licensed service providers who were part of their recovery journey.
