Each client at Aware Recovery Care is assigned a care coordinator—either a nurse or social worker—alongside two client recovery advisors (CRAs) who provide peer support and facilitate coordination throughout the treatment process. These professionals help identify any additional care required and schedule appointments with primary care providers (PCPs), therapists, medical specialists, and family education facilitators. If clients are already working with these providers, Aware Recovery Care's medical director ensures that care is well-coordinated. The family education facilitator plays a crucial role by supporting family members and educating them about addiction, recovery, and the healing process as a whole.
The treatment program at Aware Recovery Care is offered on a monthly basis, with a duration that can extend from one month to a year. Their hybrid in-home and online program allows for up to 20 virtual therapy sessions with an Internal Psychotherapist, where clients can learn effective stress management techniques and enhance their self-esteem and communication skills. These sessions also focus on recognizing and modifying negative thought patterns.
A typical day at Aware Recovery Care involves a one-hour interaction with a member of the care team, which may include a face-to-face visit from one of the two CRAs or a video consultation with a personal therapist. Clients also have monthly check-ins with their Care Coordinator, and the care team engages in regular collaboration to ensure that treatment is tailored to the clients' evolving needs and goals.
The discharge process at Aware Recovery Care is customized to fit each client’s specific treatment objectives and home circumstances. They review a thorough discharge plan with their clinical coordinator, which encompasses continued connections with support networks, personal goals, and aftercare appointments with licensed professionals from their treatment pathway.
